Social Determinants of Health

More than 80% of a person’s health is tied to factors other than clinical care including availability of or access to nutritious food, safe and affordable housing, education opportunities, reliable transportation, and community supports, also referred to as the Social Determinants of Health (SDOH). Left unaddressed, they create health inequalities which lead to worse outcomes and more expensive care. We will learn how Community Action can provide leadership on addressing the SDOH.
