Privacy Policy
The Center for Applied Management Practices owns and operates the web-based application app.elogicgenesis.com. This Privacy Policy is our shared agreement with eLogic Genesis users concerning the collection, use, and disclosure of Personal Information stored and managed through eLogic Genesis.
By using app.elogicgenesis.com, you agree to the collection and use of information in relation to this policy. The Personal Information that we collect is used for providing the services described in these policies. We will not use or share your information with anyone except as described in this Privacy Policy.
The terms used in this Privacy Policy have the same meanings as in our Software License Agreements established prior to beginning use of app.elogicgenesis.com unless otherwise defined in this Privacy Policy.
Information Collection and Security
In order to provide the services available through eLogic Genesis, information about the user, information about the user’s customers (otherwise known as ‘Clients’, ‘Patients’, ‘Participants’, etc.), and any other information collected specifically to the User and their Agency is input and stored on our servers. Our servers meet the NIST Standards for HIPAA Compliance. Our servers are hosted on the Microsoft Azure Cloud Computing Platform. User Access
Users and their Agencies are able to access and update their personal information stored in our servers as allowed by their administrative staff. eLogic Genesis Staff reserve the right to access all user information when needed to assist with issues with the Software or to provide reporting or other requested information by each agency.
Client/Patient/Participant Access
The Users and their Agency can opt to grant their clients/patients/participants access to their data via the eLogic Genesis Client Portal. By using the Client Portal, the client can interact with information in their profile for the purposes of updating existing information as requested by their User and their Agency, completing applications or assessments, and communicating with their caseworker/doctor/navigator.

HIPAA
eLogic Genesis adheres to the Health Insurance Portability and Accountability Act (HIPAA) Privacy Rule as established by the Code of Federal Regulations 45 Part 160 and Subparts A and E of Part 164. The entire rule can be found at HHS.Gov.
It is expected that all users are knowledgeable and agree to adhere to their agency’s HIPAA Guidelines as established by their agency’s HIPAA Officer.
Violations discovered to be occurring knowingly or otherwise will be addressed in accordance with eLogic Genesis’s Terms and Conditions and HIPAA Guidelines.
Information Use and Distribution
eLogic Genesis is used to track and store information collected by Users and their Agencies. The use, review, or modification of this data is forbidden except by licensed users, their agencies, or eLogic Genesis Staff. In the event that data stored within eLogic Genesis becomes subject to legal disclosure, CAMP and eLogic Genesis Staff reserve the right to adhere to all state and federal laws concerning the disclosure of personal information to law enforcement officials.
Personal information will not be distributed without written permission from the identified party.

Data Migration and Transfer
eLogic Genesis staff will not accept or send electronic files containing Protected Health Information over email or other non-secure methods. We utilize DropBox, Google Drive, or WeTransfer when sending documents that contain sensitive information.

Network Referrals
Entities have the option to send or receive “Network Referrals” where client data will be transferred between two entities that operate separate databases within eLogic Genesis. The intention of a Network Referral is to provide client management services while maintaining separate but similar client records. This data sharing can enhance the Users’ experience by allowing access to multiple agencies through our platform.
Data shared in a Network Referral may include but is not limited to:
- Name (first, last, sur)
- Date of birth
- Postal address
- Social security number
- Email address
- Race/ethnicity
- Housing (size, type)
- Education
- Budget/financial
- Legal records
- Employment
- Personal contacts
- Health information including:
- General health
- Disability status
- Substance and/or domestic abuse history and treatment
- HIV/AIDS Status
- Medical History
- Service history
- Case history notes
- Other documents provided to Users and their Agencies
Data shared in a Network Referral will be governed by agreements (MOUs, ROIs, etc.) between the agencies sending and receiving the Network Referral within eLogic Genesis. Any information or agreements pertaining to the sharing of confidential information described in these agreements is not governed by eLogic Genesis or CAMP staff.
Local Storage
eLogic Genesis makes use of local storage for a variety of reasons which help improve the user experience. Local storage allows data to be stored in your browser and is used to identify you. Genesis uses local storage to identify the current user as well as additional settings to simplify the use of the software.
Additionally, local storage is used by the ZenDesk web widget but does not collect or store any data pertaining to the user.

Children’s Privacy Policy
Person’s ages 18 and younger are strictly prohibited from using the Software.
eLogic Genesis can be used to track information for person 13 years of age or younger. In the event that a client wishes to disclose information about their children, for example to a caseworker to establish eligibility for a government program requiring household information, they have willfully consented to the collection, storage, and responsible use of that PI by both the agency they’re working with as well as eLogic Genesis staff.

Information Collection
Information collected for children is input by users working directly with the child and/or their parents or guardians. It is our expectation that the user and their agency have established policies and procedures for their management of children’s data within the eLogic Genesis system.
Information Use
Children’s data stored in eLogic Genesis is utilized for various purposes including informing reports queried by the agency managing the child’s client profile, tracking data over time, and updating contact information. These functions are limited to the person(s) responsible for directly managing that electronic profile.
eLogic Genesis staff will not access children’s profiles for any reason unless it is necessary for technical issue resolution.
Information Disclosure
We do not, for any reason, disclose information of any client, age 13 years or younger, without the express written permission of the parent/legal guardian.
In the event that law enforcement is requesting data regarding children, appropriate actions will be taken to provide the data in a timely and law-abiding manner.
Parent Rights
eLogic Genesis will never prompt the user or child to supply more information than is necessary for the user to carry out the functions of their position.
Parents reserve the right to request data about their child(ren) for review, to revoke further data use, or to request to delete information stored about their child(ren).
Changes to this Privacy Policy
We may update our Privacy Policy from time to time. We advise you to review this page periodically for any changes. We will notify you of any changes by posting the new Privacy Policy on this page. These changes are effective immediately once they are communicated to our users.

Terms and Conditions
The Center for Applied Management Practices (CAMP) owns and operates the web-based application eLogic Genesis (app.elogicgenesis.com). These terms and conditions describe acceptable usage of eLogic Genesis. Please read these terms and conditions carefully before utilizing eLogic Genesis’s services.
By using eLogic Genesis, you agree to adhere to these terms and conditions. The Personal Information that we collect is used for providing the services described in these terms and conditions. We will not use or share your information with anyone except as described in these terms and conditions.
The terms used in these terms and conditions have the same meanings as in our Software License Agreements established prior to you and your agency beginning use of eLogic Genesis unless otherwise defined in these terms and conditions. Your continued access or use of eLogic Genesis shall be deemed your acceptance of these terms and conditions.
Any active license agreements signed and agreed upon prior to 1/1/2020 are grandfathered and honored until the date those license agreements are complete.
Definitions
Software
The term “Software” shall mean the application found at app.elogicgenesis.com. The term “Software” includes any corrections, bug fixes, enhancements, updates, or other modifications, including custom modifications to such computer programs and user manuals.
User
The term “User” shall mean any person granted a license to operate eLogic Genesis for any purpose. “User and their Agency” shall mean the user and the Agency they received their license through.
Authorized Users
Each User is granted one license. The User is not to share, store, or make access to their account available or obvious to other persons not authorized to use the Software.
Outcome Scales/Matrices
eLogic Genesis comes pre-loaded with content in the form of Outcome Scales and Matrices. The term “Outcome Scales” shall mean a continuum that describes different states or conditions of status used to identify and measure incremental change. The Outcome Scales use a five-point scale, or benchmarks: Thriving, Safe, Stable, Vulnerable, and In-Crisis or similar lexicon to measure incremental change for clients. Outcome Scales are used for client assessment and follow-up. When more than one outcome scale is used, it becomes an Outcome Matrix. All Outcome Scales/Matrices are organized by ten domains: Education and Training (includes Childcare), Employment, Financial Literacy, Food and Nutrition, Health, Housing, Human Services, Income, Childcare, and Transportation. All the primary or base Outcome Scales/Matrices are the intellectual property of and copyrighted by CAMP.
No part of the primary or base Outcome Scales/Matrices or Survey Instruments may be reproduced in any form or by any means, mechanical or electronic, or by any information storage or retrieval system or other methods, for any use, without written permission of The Center for Applied Management Practices.
Information Collection and Use
In order to create and maintain user access, eLogic Genesis requires the user to maintain a profile within eLogic Genesis which requires an email address to establish a username. Other identifiable information, including but not limited to your name and phone number may be included in the user profile. The information that we collect may be used to contact you or identify certain activities logged by your user account.
Confidential Information
User acknowledges that the Software contains proprietary information, including trade secrets, know-how, and confidential information that is the exclusive property of eLogic Genesis. During the period these terms and conditions are in effect, and at all times after its termination, User shall maintain the confidentiality of this information and not sell, license, publish, display, distribute, disclose or otherwise make available this information to any third party nor use such information except as authorized by these terms and conditions. User shall not disclose any such proprietary information concerning the Software, including but not limited to any flow charts, logic diagrams, user manuals, and screens, to persons not authorized in the Software License Agreement without the prior written consent of eLogic Genesis staff. eLogic Genesis acknowledges that all client information contained in the software is confidential. eLogic Genesis, its employees, agents, and all other persons who are permitted to access the database’s confidential data shall maintain the confidentiality of this information and not sell, license, publish, display, distribute, disclose or otherwise make available this information to any third party. If eLogic Genesis wants access to the data for research purposes, it will obtain written permission from the User and their Agency and eLogic Genesis will not disclose any data that can be identified as created by or belonging to the User or their agency.

Third Party Disclaimer
eLogic Genesis does not and cannot control the flow of data to or from the software and other portions of the user’s network and the Internet. Such flow depends on the performance of the network or internet services provided or controlled by users or third parties. At any time, actions or inactions of User or such third parties can impair or disrupt User’s connections to the user’s network or the Internet (or portions thereof). Accordingly, eLogic Genesis disclaims any and all liability resulting from or related to such.
Friendly Use Environment
As a condition of using eLogic Genesis, the User agrees not to use the information in the database for any purpose that is unlawful or prohibited by these terms. Additionally, the User agrees not to use eLogic Genesis for any not reasonably intended purpose including the following:
- To harass, abuse, threaten, or intimidate any person;
- To communicate with eLogic Genesis Staff or any other users in an abusive or offensive manner;
- To post or transmit, or cause to be posted or transmitted, any abusive, offensive, profane, pornographic, libelous, defamatory, or obscene information that infringes on any rights of any person;
- For posting, viewing, or any other use of information that is not permitted under the laws of the jurisdiction where User accesses and uses eLogic Genesis;
- To share access to eLogic Genesis with any personnel other than appropriate agency personnel or eLogic Genesis Staff.
These and other situations determined to be a violation of these terms and conditions can be used as cause for immediate termination of services with or without warning communicated to the user. eLogic Genesis reserves sole discretion when determining whether or not a User violated these terms and conditions.
Client Portal
eLogic Genesis users have the ability to create accounts for their clients to access the eLogic Genesis Client Portal. Any accounts created and the actions taken by the account holders are subject to the terms of our Client Portal Terms and Conditions of Use, Privacy Policy, HIPAA Policy, as well as the terms provided in this document.
Any person granted access to the eLogic Genesis Client Portal is willingly providing personal information and updates to the system which are not monitored by CAMP or eLogic Genesis employees. Given the sensitive nature of this information, users and their clients will establish, maintain, and uphold Release of Information terms prior to allowing access to and using the Client Portal.
In certain circumstances, users and their agencies may store the release of information within eLogic Genesis. The terms and conditions in the Release of Information form NOT provided directly by CAMP or eLogic Genesis employees are not covered by these Terms and Conditions and should be considered a separate agreement between the user and the client they are providing access to the system.

Termination
eLogic Genesis Staff shall have the right to terminate the User’s use of the Software at any time. In the event the User violates any material provision of these terms and conditions, or In the event, the User (i) terminates or suspends its business; (ii) becomes subject to any bankruptcy or insolvency proceedings under federal or state statutes; (iii) becomes insolvent or subject to direct control by a trustee, receiver or similar authority; or (iv) has wound up or liquidated its business, voluntarily or otherwise.
Upon the occurrence of an Event of Default, the User and their Agency shall deliver to the defaulting party a notice of intent to terminate that identifies in detail the Event of Default. If the Event of Default remains uncured for thirty (30) days, eLogic Genesis Staff may enact these terms and conditions to terminate the license granted to the User and their Agency by delivering to the defaulting party a notice of termination that identifies the effective date of the termination, which date shall not be less than thirty (30) days after the date of delivery of the notice of intent to terminate. In the event, additional time is needed to make payment on the license(s) CAMP must be notified within the 30-day window.
If this Agreement is terminated due to an Event of Default and results in damages to the non-defaulting party, the non-defaulting party may recover from the defaulting party all damages that are sustained, including reasonable attorney’s fees, costs, and expenses.
Ownership
The data stored in eLogic Genesis is the property of the User’s Agency. CAMP has no ownership or other interest in the stored data in the database. In the event of termination, eLogic Genesis shall provide a complete extract of all the Users and their Agency’s proprietary data including but not limited to a .csv file of all client records to include all available data fields (demographic and intake fields, employment details, all assessment history, and income/expense detail, a comprehensive list of custom fields and field values, and an extract of all User and their Agency-specific assessment outcome scales) delivered within 30 days following termination of this contract. Users and their Agency will continue to have full access to eLM to continue its operations until all data files are delivered.
Assignment
User shall not assign or otherwise transfer the Software to anyone, including any parent, subsidiaries, affiliated entities or third parties, or as part of the sale of any portion of its business, or pursuant to any merger, consolidation, or reorganization, except as set forth herein, without prior written consent from eLogic Genesis Staff.
Indemnity
User and their Agency shall indemnify the Software, its officers, employees, and agents, for all damages, costs, and expenses, including attorney’s fees, arising from personal injury or property damage to third parties occurring as a result of the use of the Software by User, including but not limited to the making, using, selling, or exporting of products, processes, or services derived therefrom. This indemnification shall include, but not be limited to, indemnification for any resulting liability. User’s indemnification obligations arising hereunder shall apply irrespective as to whether such damages, costs, and expenses arise or are alleged to arise, directly or indirectly, from the participation, whether active or passive or negligence, whether ordinary or gross, of such parties, their employees, agents or other personnel.

CAMP and eLogic Genesis are committed to the protection and security of the data stored on our servers, thus, we have enacted the following Technical, Physical, and Administrative safety standards which adhere to HIPAA standards.
Technical safeguards
Data is encrypted (to NIST standards) at rest and in transit once it leaves our servers.
Roles and Privacy groups can be established to prevent sharing of data between coworkers with differing levels of responsibility within the software. There is an option to allow administrators to turn off sharing of information within user accounts.
eLogic Genesis will time out after 4 hours of inactivity. Every 12 hours, users will be required to sign back in.
Additional tools for encryption/decryption are the responsibility of users, not eLogic Genesis or CAMP.
Physical Safeguards
Facility access controls reasonably limit who has physical access to data stores. We lock all entryways into our facilities while we’re there and when we’re not. We completely log off and shut down all computers at the end of each day.
We request that each agency using Genesis enact policies to regulate the placement of computer monitors, fax machines, and other mediums that display PHI. If access is granted to users on their personal mobile devices, a plan must be implemented to ensure total destruction of PHI from devices in the event of a breach of security.
Inventory of hardware used to facilitate PHI is kept and maintained by eLogic Genesis Staff. We follow the physical security best practices including locking our laptops in secure facilities when not in use and logging out of eLogic Genesis while not using the application.
Administrative Safeguards
We perform risk assessments on an annual or as-needed basis. Documentation of those updates is recorded under “Historical Risk Assessments” below.

Violations
Violations of these policies and procedures committed by CAMP/eLogic Genesis Staff (willful or unintended) can involve disciplinary action up to termination.
Violations of these policies and procedures by users (willful or unintended) will be subject to our Terms and Conditions.
Training
CAMP/eLogic Genesis Staff HIPAA Training occurs annually for current employees (a record of staff agreements to comply is stored in each staff member’s user profile in eLogic Genesis). New hires will be trained within 2 weeks of onboarding. Additional training will be provided if HIPAA policies are updated or if an infraction occurs.

Records are maintained and stored in our database for the duration of activity as defined by our users. eLogic Genesis retains the information stored on our servers in perpetuity until removal is initiated by or requested by our users. Contingency plan for breaches in data security
Users will be notified as soon as any breaches are discovered.
eLogic Genesis Developers will take all reasonable and possible steps to recover any compromised data. Further action taken will be dependent on the severity of the breach.
Please see our Cyber Security Incident Response Protocol for more information about how we handle data breaches.
CAMP/eLogic Genesis staff will make reasonable efforts to notify all users within 30 days of the initial discovery of the breach. We will notify users of the nature and extent of the data which was breached as well as provide instructions on any requisite procedures such as resetting passwords and/or updating security questions.
Pursuant to Colorado HB 1128 – Colorado Consumer Privacy Protection Act
In the event that a breach affects the information impacting more than 500 Coloradans, we will notify the Colorado Attorney General’s office.
All compromised information will be disposed of promptly and securely using the most up-to-date methods available at the time of the breach.
Children’s Privacy Policy
Person’s ages 18 and younger are strictly prohibited from using the Software.
eLogic Genesis can be used to track information on person 13 years of age or younger. In the event that a client wishes to disclose information about their children, for example to a caseworker to establish eligibility for a government program requiring household information, they have willfully consented to the collection, storage, and responsible use of that PI by both the agency they’re working with as well as eLogic Genesis staff.
Information collected about children 13 years old and younger include but are not limited to:
- Name (first, last, sur)
- Date of birth
- Postal address
- Social security number
- Email address
- Race/ethnicity
- Housing (size, type)
- Education
- Budget/financial
- Legal records
- Employment
- Personal contacts
- Health information including:
- General health
- Disability status
- Substance and/or domestic abuse history and treatment
- HIV/AIDS Status
- Medical History
- Service history
- Case history notes
- Other documents provided to Users and their Agencies
- Includes photographs used for identification purposes
Information Collection
Information collected for children is input by users working directly with the child and/or their parents. It is our expectation that the user and their agency have established policies and procedures for their management of children’s data within the eLogic Genesis system.
Information Use
Children’s data stored in eLogic Genesis is utilized for various purposes including informing reports queried by the agency managing the child’s client profile, tracking data over time, and updating contact information. These functions are limited to the person(s) responsible for directly managing that electronic profile.
eLogic Genesis staff will not access children’s profiles for any reason unless it is necessary for technical issue resolution.
Information Disclosure
We do not, for any reason, disclose information of any client, age 13 years or younger, without the express written permission of the parent/legal guardian.
In the event that law enforcement is requesting data regarding children, appropriate actions will be taken to provide the data in a timely and law-abiding manner.
Parent Rights
eLogic Genesis will never prompt the user or child to supply more information than is necessary for the user to carry out the functions of their position.
Parents reserve the right to request data about their child(ren) for review, to revoke further data use, or to request to delete information stored about their child(ren).
